Indicator Purpose

This indicator demonstrates community-based care, incorporating reintegration into the community and prevention of residential care. Showcasing stronger community support systems that help alleviate pressure and support families to be more stable and resilient as they care for vulnerable people.

Indicator Guidance

Definition of Concept

  • Stability: in family dynamics refers to a consistent, predictable environment where members feel safe, supported, and valued. It encompasses emotional, psychological, and financial steadiness that helps individuals navigate life’s challenges together

Survey Question

Question is answered by the service care provider (ex/case manager, social worker, nurse, etc.) that is providing ongoing community-based care to individuals and families.  

  • In the past six months have you seen a change in stability through ongoing community-based care? (Much less stable / Less stable / No change / More stable / Much more stable)

Scoring
